The order of execution

 

- Applying at PAA (No. 5, Nguyen Trai Street, Lien Bao Ward, Vinh Yen City, Vinh Phuc Province).

- Besides applying directly at PAA, applicants for judicial review cards can be implemented in two ways:

+ Register for the online judicial record card;

+ Register for the judicial record card through postal service.

- Receive results directly at PAA or via postal service at registered address.

Composition, number of records

 

- A declaration requesting the issuance of a judicial record card (Form No. 03/2013 / TT-LLTP).

- A copy of the identity card or passport of the person granted a judicial record card; Copies of household registration book or temporary residence book or certificate of permanent residence / temporary residence of the person issued a judicial record card. (In case of filing a copy, the original must be presented for comparison. For comparison, the authenticated copies shall be submitted according to the provisions of law.

- A power of attorney authorizing others to fill in the procedures for requesting the issuance of judicial record card No. 1 (if the authorized person is the father, mother, spouse or children of the authorizing person, authorization documents). Authorization documents must be notarized or authenticated according to the provisions of Vietnamese law. 

Individuals requesting the issuance of judicial record card No. 2 shall not authorize other persons to carry out the procedures for requesting the issuance of judicial record cards.

- A copy of the identity card or passport of the authorized person (in case of authorization to request the issuance of judicial record card No. 1)

In addition, applicants for judicial record cards who are entitled to exemption or reduction of fees must present papers to prove.

Period of settlement

 

Within 10 days from the date of receipt of a valid request. If the person requesting the issuance of a judicial record card is a Vietnamese citizen who has resided in many places or has a period of residence abroad, the foreigner must prove that the condition is automatically annulled. The time limit shall not exceed 15 days.

Fee, charge

 

- Fee for issuing judicial record card: 200,000 VND / time / person.

- The fee for granting judicial record cards to pupils, students and people with meritorious services to the revolution, relatives of martyrs: VND 100,000 / time / person.

In cases where a person granted a judicial record card requests the granting of more than 2 tickets at a time, the fee for the third-level ticket onwards shall be collected by the agency issuing the judicial record card to VND3,000 / card for the printing fee of the judicial record.

- Persons of poor households according to the provisions of law; residents of communes having special difficulties as prescribed by law shall be exempt from the fee for granting judicial record cards.

- Declaration requesting the issuance of a private report card (Form No. 03/2013 / TT-LLTP);

- A declaration requesting the issuance of a judicial record card (used for individuals in case of authorization of the request for the issuance of judicial record card No. 1 and an individual who is the father or mother of a minor who requests the granting of a private examination card.
